Lead the Care That Defines Us
Great hospice care doesn't happen by accident — it's built by leaders who set the standard, develop their teams, and never lose sight of the patient at the center of it all. At Transitions Hospice, our Director of Nursing is that leader.
This is a role for a seasoned RN who's ready to step beyond the bedside and shape the clinical culture of an entire region. You'll guide interdisciplinary teams, drive quality outcomes, and ensure that every patient in our care receives the compassionate, dignified experience that defines the Transitions difference.
As Director of Nursing, you'll own the clinical integrity of regional hospice services — from care planning and quality improvement to team development and operational execution. Your responsibilities span four key areas:
Transitions isn’t just a place to manage — it's a place to lead with purpose. We've built an organization where clinical excellence and human compassion go hand in hand, and we're looking for a Director of Nursing who shares that conviction. You'll have the autonomy to make a real impact, the support of a mission-driven team, and the satisfaction of knowing your leadership directly shapes how patients experience the end of life.
