Job summary

Diversicare Healthcare Services, LLC seeks a Director of Care Coordination to lead and optimize care delivery for patients and residents. You will coordinate restorative nursing, oversee daily care coordination meetings, and ensure effective communication with families and healthcare teams.

The role requires strategic leadership, strong regulatory knowledge, and experience in case management to drive discharge planning and quality outcomes in a dynamic clinical setting.

Qualifications

• Degree in nursing, therapy or other related clinical field with a current license.
• Knowledge of healthcare reimbursement, utilization management, and discharge planning.
• Experience providing strategic leadership, managing teams and leading projects.
• Ability to formulate and implement short- and long-term strategic plans.
• Strong verbal, written and interpersonal skills.
• Proven ability to lead a team to achieve outcomes.
• Knowledge of PDPM requirements and regulatory standards.

Responsibilities

• Coordinate the restorative nursing program and integrate it with other departments.
• Facilitate daily care coordination meetings and ensure timely communication with patients and families.
• Lead care management activities including Engage/72 HR Admission meetings.
• Collaborate with clinical teams to develop and execute care plans.
• Coordinate discharge planning and post-discharge communication (home health, DME).
• Monitor quality measures with care coordination team.
• Ensure accurate Medicare and Managed Care documentation per guidelines.
• Review ADRs to ensure completeness of documentation.

Responsibilities

• Facilitate daily care coordination meetings to align care resources with patient needs and benefits.
• Participate in and facilitate Engage/72 HR Admission meetings as part of the care management system.
• Manage the restorative nursing program, integrating it with the activity department.
• Ensure timely communication with patients and families regarding changes in condition and discharge planning.
• Collaborate with the clinical team to develop and execute care plans.
• Coordinate discharge planning and post-discharge communication, including home health and DME.
• Monitor Quality Measures alongside the care coordination team.
• Serve as a skilled care documentation specialist, ensuring accurate and timely completion of Medicare and Managed Care regulatory guidelines.
• Review Additional Documentation Requests (ADRs) to ensure all documents meet the request.

Qualifications

• Degree in nursing, therapy or other related clinical field (i.e. social services), must hold a current license in respective field.
• Knowledge of healthcare reimbursement, utilization management, and discharge planning.
• Experience providing strategic leadership, managing teams and leading projects required.
• Ability to formulate and implement short and long term strategic plans
• Strong verbal, written and interpersonal skills.
• Must have proven ability to lead a team to achieve desired outcomes of team
• Strong communication and organizational skills required
• Must have knowledge of PDPM requirements of insurers
