Job summary

Avera Health in Marshall, MN seeks a senior nursing leader to oversee total nursing care across residents and ensure accurate nursing records while guiding policy development and staff orientation. You will lead the MDS/Care planning, staffing, budgeting, safety, and the quality improvement (QAPI) program, coordinating with physicians and administrators to plan care and discharge strategies.

Qualifications

  • RN license active in practicing state upon hire.
  • 4-6 years in long term/skilled care or similar.
  • 1-3 years of leadership experience.

Responsibilities

  • Total nursing care of residents and accuracy of nursing care records.
  • Oversee admissions, MDS/Care planning, restorative nursing and care delivery.
  • Develop and implement policies for total care, including social services and delegated medical care.
  • Determine staffing levels with Administrator to ensure 24-hour care.
  • Plan, budget and participate in facility decisions.
  • Coordinate safety and lead the QAPI program with staff participation.
  • Delegate non-nursing tasks to appropriate staff per policies.
  • Coordinate Medicare/Medicaid coverage requirements.
  • Ensure a comprehensive plan of care for each resident and plan discharges.
  • Hire, train, develop and retain staff; supervise performance and resolve issues.

A Brief Overview

Responsible for providing care and services to meet the needs of the assigned facilities through strategic and accountable leadership. This includes the establishment, implementation and adherence to policies and procedures, coordination of orientation programs for new staff, development and engagement of existing staff and is instrumental in leading quality improvement activities; develops monitoring tools, collects data, uses data to drive improvements in care and services.

What you will do
  • Responsible for total nursing care of residents and the accuracy of the nursing care records. Participates in the selection of prospective residents based on nursing care needs and nursing personnel competencies available.
  • Oversees all Federal/State SNF Conditions of Participation including admissions, MDS/Care planning process, restorative nursing and care delivery.
  • Establishes and implements policies/procedures for the total care of residents inclusive of nursing activities, social services, and delegated medical care.
  • Determines, along with the Administrator, the numbers and levels of nursing personnel. Establishes staffing pattern to provide quality nursing care 24 hours/day.
  • Participates in planning, decision making, and budgeting for the facility.
  • Remains aware of the safety needs for staff and residents; develops policies relevant to quality of care. Coordinates the QAPI program and facilitates staff participation in QAPI.
  • Delegates and monitors non-nursing responsibilities to other staff consistent with their training, experience, competence, and legal authorization and with nursing home policy.
  • Understands/coordinates Medicare/Medicaid coverage requirements.
  • Assures that a comprehensive plan of care is established and implemented per regulations for each resident. Interacts with the physicians/providers to plan care for the residents. Assures that discharge and transfer planning for residents is conducted.
  • Responsibilities include interviewing, hiring, developing, training, and retaining employees; planning, assigning, and leading work; appraising performance; rewarding and coaching employees; addressing complaints and resolving problems.
