Job summary

You will document ISP outcomes, support communication and self-advocacy, and report changes to leadership. CPR/First Aid certification and valid work authorization are required, with a minimum of 1 year healthcare experience.

Qualifications

• CPR/First Aid certification is required.
• Authorization to work in the U.S.
• Clearance of background check
• High School Diploma or equivalent
• Minimum 1 year of healthcare experience
• Patience, empathy and a desire to support individuals with intellectual disabilities
• Excellent interpersonal and communication skills
• Ability to work independently and as part of a team
• Flexibility to work varied shifts (days, evenings, weekends)

Responsibilities

• Assist individuals with Activities of Daily Living (ADLs) including grooming, cooking, cleaning and personal hygiene.
• Support individuals in accessing and participating in community activities and recreation.
• Accompany individuals to appointments, shopping or social outings as needed.
• Encourage skill-building for community engagement (money management, transportation use, social interaction).
• Implement and document individualized support plans and ISP outcomes.
• Teach and reinforce skills that promote independence (cooking, budgeting, personal safety).
• Support communication and self-advocacy efforts.
• Report any incidents, changes in condition, or concerns to the supervisor promptly.
