Prairie Community Services in Thief River Falls, MN is seeking a Direct Support Provider for the Hanson Home. This part-time, on-site role involves supervising and supporting individuals in daily living activities, recreation and community orientation.

The starting wage is $17.50-$18.50 per hour. You will operate in a mission-driven nonprofit focused on integrity, respect and service, with training provided to ensure success.

Qualifications

• Minimum age 17+ or as required by license.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Read, write, and understand English in all business contexts.
• Read and follow each person served’s plans and procedures.
• Deliver support with person-centered approach.
• CPR/First Aid administered when needed.
• Chart daily program information.

Responsibilities

• Supervise, care and ensure safety of individuals served in daily living, recreation, and community activities.
• Support with medication management, meal preparation, laundry and household tasks.
• Assist with transportation and community orientation as required.
• Document progress and daily activities in the individual’s plan.

Description

As a Direct Support Provider, you will provide supervision, care and safety to individuals being served that include, but are not limited to, areas of daily living, recreation, community orientation, medication management, meal preparation, laundry, and cleaning. Starting wage $17.50-$18.50 per hour.

Qualifications:

• Minimum of 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.
