Job summary

Prairie Community Services in Warroad, MN invites a Direct Support Provider to join our on-site Elkwood Home team. You will supervise and care for individuals served, support daily living activities, meals, recreation and community integration, and assist with medication management as needed.

Starting wage is $16.75-$18.75 per hour, with benefits and opportunities to grow within our mission-driven nonprofit.

Qualifications

• Minimum age 17 or as dictated by licensure.
• Must pass DHS background clearance.
• Must have a valid driver’s license.
• Must be able to read, write, communicate and understand English.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Description

As a Direct Support Provider, you will provide supervision, care and safety to individuals being served that include, but are not limited to, areas of daily living, recreation, community orientation, medication management, meal preparation, laundry, and cleaning. Starting wage is $16.75-$18.75 per hour.
