Job summary

Sevita's ResCare Community Living in Denton, TX is seeking a Direct Support Program Coordinator to help individuals live more independently through person-centered services.

You will develop and implement treatments, access medical services, and work with a treatment team. Requires a bachelor''s degree if state requires, three years of related experience, and a valid driver''s license. This is a full-time role, Monday–Friday, 8 a.m. to 5 p.m., paying $20 per hour with benefits.

Responsibilities

• Assist in accessing medical and health-related services for individuals
• Assess referred individuals and help develop a service plan with the team
• Implement and contribute to development and revision of programs and behavior plans
• Provide monitoring and support to mentors and staff
• Assist manager/director with supervision of staff
• Provide leadership and direction in the absence of management

Hours: Mon - Fri 8a-5p

Requirement: Current driver's license with 1 year driving history without restrictions

Direct Support Program Coordinator

As a Direct Support Program Coordinator, you will have the opportunity to have a direct impact on the individuals we serve by developing and implementing unique treatments to provide to them.

• Assist in accessing medical and health-related services for the individuals that receive our services
• Assess referred individuals and participate in the development of a service plan in conjunction with a treatment team and funding source
• Implement and actively contribute to development, review, and revision of programs and behavior intervention plans
• Provide monitoring, consultation, and comprehensive support services to Mentors and staff regarding work with individuals supported
• Aid manager/director with direction, supervision, and oversight of assigned staff
• Provide effective leadership and direction to staff in the absence of manager/director

Qualifications

• Bachelor’s degree if required by state
• Three years of related work experience
• Valid driver's license in good standing and car registration/auto insurance (if driving for company)
• A reliable, responsible attitude and a compassionate approach
