Job summary

Samaritans-At-Last is seeking a Direct Support Professional Team Lead in Boothwyn, PA. The role focuses on overseeing health, safety and well-being, implementing ISP/BSP/Crisis plans, and guiding individuals toward independence.

You will document progress and ensure adherence to policies in all settings. Responsibilities include supervising services, assisting with daily living tasks, coordinating activities, and facilitating community-based experiences while maintaining strong communication

Qualifications

• Two years of prior Direct Support Professional experience.
• High school diploma or GED.
• CPR and First Aid certification; valid driver’s license.
• Full Work Physical with PPD Screening.

Responsibilities

• Provide supervision and guidance for individuals receiving services in homes, community, or residential settings.
• Carry out services and goals set forth in individual support plans.
• Assist with daily living activities, including meals and personal hygiene.
• Provide recreational and social activities support at the individual’s home or community.
• Accompany individuals to appointments, classes, or activities outside the home.
• Transport individuals to activities within the department mileage cap.
• Develop rapport with individuals and maintain a safe, supportive relationship.
• Document service notes daily and report incidents to management.

Direct Support Professional Team Lead

Salary Range: $14.00 To $15.50 Hourly

SUMMARY:

Ensure the health, safety and well-being of the individual they offer services to, through supervision and assistance. To carry out the terms set forth in the individual’s ISP (Individual Support Plan), Behavior Support Plan (BSP) and or Crisis Plan. Assist and support in the planning, implementation, and documentation of programs designed to meet the social, emotional, physical, and personal needs of Individuals. Assist, support, and instruct Individuals in self-help skills designed to encourage independence. To maintain a safe working environment, comply and adhere to company’s and program policies and procedures at all times.

DUTIES AND RESPONSIBILITIES:

• Provide supervision and guidance for individuals receiving services in their homes, community or residential settings.
• Carry out the services and goals set forth in the individual support plans
• Assist with daily living activities, including preparing meals and assisting with personal hygiene, etc.
• Provide recreational and social activities support, including volunteer or civic-minded opportunities at the following locations:
  • At the individual's home.
  • In the individual community or proximity to his/her community
• Accompany individual to appointments, classes, or other activities outside the home.
• Transport individuals to Outcome and Goal related activities that are within the departmental mileage cap and are part of goal attainment.
• Develop a rapport with individuals and cultivate a safe and supportive relationship
• Document service notes on a daily basis, reporting incidents to management as they occur
• Maintain a positive, respectful relationship with individuals and their families
• Performs other related duties as assigned by management.

Requirements:

• Must have at least two (2) years of prior experience as a DSP or equivalent working with adults in the intellectual disability/development and autism field.
• High school diploma or general education degree (GED)
• Active CPR and First Aid Certification required; Valid Driver's License required
• Full Work Physical with PPD Screening
