Qualifications

• High School Diploma or equivalent required.
• Six months of experience in human services, direct care, or care coordination preferred.
• Strong communication, adaptability and teamwork skills.
• Valid driver’s license and access to a vehicle with insurance.

Responsibilities

• Provide person-centered support to participants in home and community settings.
• Foster meaningful relationships and assist with goals like budgeting, exercise, and nutrition.
• Promote independence and community participation for participants.
