Lifetime Assistance, Inc. is seeking a Direct Support Professional – Per Diem I to join our on‑site team in Rochester, NY 14624. You will assist individuals with daily living activities, support goals, and promote independence in community settings.

Starting wages are $17.44/hr for Day Services and $18.47/hr for Residential care. The role requires flexibility across sites and shifts, with a minimum of 16 hours every two weeks.

• Ability to follow individualized care plans and maintain required supervision levels.
• Basic computer skills for documentation and communication.

Responsabilités

• Provide respectful personal care including bathing, grooming, toileting, dressing and meal support.
• Support individual goals, independence, and skill development while promoting community inclusion.
• Implement individualized plans including IPOP, SAP, BSP, and PONS.
• Maintain accurate documentation in the Electronic Health Record (EHR).
• Ensure health, safety, dignity, and well‑being at all times, reporting concerns immediately.
• Foster positive communication with individuals, families, co‑workers, and supervisors.
• Drive agency vehicles to transport individuals to appointments, programs, or community outings.

Your Core Responsibilities:

• Ability to lift/move up to 35 pounds and perform physical care responsibilities.
• Required: Maintain certifications in SCIP-R and First Aid/CPR (training provided).
• Must pass required background checks and clearances (SCR, fingerprinting, SEL, MHL).
• Preferred: Prior experience in human services or direct care.
