Job summary

Citizens Options Unlimited seeks a dedicated Direct Support Professional to help people live active, independent lives in their homes and communities. You will support daily activities such as self-care, meals, transportation, and safety while following individualized plans and documenting progress.

Colleagues collaborate with nurses and supervisors to ensure quality care, safety, and wellbeing. The role requires a NYS driver’s license and willingness to complete AMAP training within six months

Qualifications

• High school diploma or equivalent preferred.
• Qualified NYS driver's license required and maintained throughout employment.
• Ability to communicate and understand written and oral directives.
• AMAP Training is required and must be completed within six months of hire.
• First Aid/CPR/SCIP-R Training is preferred.

Responsibilities

• Ensure a safe and clean home by supporting household chores and participating as needed.
• Follow treatment plans and use adaptive/safety equipment to promote independence and wellbeing.
• Assist with meal preparation, teaching diet adherence and portion control.
• Assist with medication administration according to AMAP policy.
• Coordinate with residential nurses for accurate medical documentation.
• Document treatment plans and goals to help clients achieve independence.
• Attend meetings to discuss progress and gather input.
• Provide transportation as needed for emergencies.
• Mentor new staff by guiding routines and training materials.
• Perform nightly sleep checks and participate in emergency evacuations.

Overview

Practices and promotes the agency’s mission statement and promotes the agency’s guiding principles in every interaction. Provide ongoing support to people in becoming active participants in every aspect of their lives (i.e., self-care routines, health, safety, household chores, socialization, and overall wellbeing) and to ensure that a high quality of life is maintained.

Responsibilities

Primary duties and responsibilities include but not limited to:

• Ensures a safe and clean home by providing support to people to assist them with the completion of household chores and by participating in said household chores as needed
• Follows all treatment plans and utilizes all corresponding adaptive and safety equipment needed for each person to promote independence and ensure their health, safety and wellbeing.
• Supports people in preparing meals by teaching concepts such as diet adherence, portion control and cooking skills
• Assists people with medication administration according to the AMAP policy and procedures
• Works with residential nurses to ensure accurate and current medical documentation for people supported if maintained.
• Implements and documents all treatment plans and other agency related materials (i.e., residential habilitation goals, medical case notes, communication logs, etc.) to assist people in achieving independence in their selected valued outcomes and to ensure their health, safety and well-being
• Adheres to overtime policy and procedures to ensure that quality support is provided to people each day
• Attends all necessary meetings to discuss each person’s progress and goal achievement and offer and gather information and ideas
• Provides transportation as needed for emergency situations
• Mentors new Direct Support Staff by demonstrating supportive routines and by reviewing training manuals to ensure new employees are sufficiently groomed to complete responsibilities.
• Responsible for documenting sleep checks through the night for the people supported
• Responsible for knowing and participating in all emergency evacuation procedures
• Performs all other job-related duties as requested by supervisor

This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check

Qualifications

• High school diploma or equivalent preferred.
