Job summary

SCO Family of Services is seeking staff to provide close supervision for individuals per each treatment plan, participate in program meetings, and maintain orderly care. A clean NYS driver’s license is preferred, with experience in developmental disabilities or brain injuries encouraged.

Responsibilities include safe transport, documentation, and ensuring a safe, healthy environment while complying with OPWDD regulations.

Qualifications

  • Requires a minimum high school diploma or GED.
  • Clean and valid NYS driver’s license preferred.
  • Experience working with people with developmental disabilities or traumatic brain injuries preferred.

Responsibilities

  • Provide close supervision for individuals per their treatment plan and transfer forms for 1:1 status.
  • Participate in program planning meetings and sign off on treatment plans.
  • Maintain individuals’ possessions neatly and report needs to management.
  • Transport individuals safely; maintain a valid NYS Driver’s License and complete vehicle logs.
  • Keep the facility and property clean and safe; report issues to management.
  • Perform all other duties to ensure individuals are safe, healthy, and comfortable.
  • Comply with OPWDD regulations and agency procedures.
  • Obtain and maintain SCIP-R certification; attend mandatory trainings.
  • Obtain and maintain AMAP certification and submit copies to relevant programs.
  • Attend staff meetings and demonstrate competency in required training areas.

Job description

Responsibilities
  • Provide close supervision for individuals in accordance with the individual treatment plan. If an individual is on a 1:1 supervision status, provide continual supervision as noted in the treatment plan and complete the transfer form when transferring the individual’s care to another staff.
  • Participate in the individuals' program planning meetings; review and sign off on Annual and Semi-Annual Treatment Plans.
  • Maintain individuals’ possessions in a neat and orderly manner; communicate needs for the individuals to management and complete documentation in accordance with program procedures.
  • Transport individuals in a safe and comfortable manner; follow all NYS laws and agency procedures, maintain a valid NYS Driver’s License, and complete all documentation including vehicle logs in a timely and complete manner. Communicate the condition of vehicles to management and ensure that the individuals have an optimum experience by not having the radio too loud and maintaining a comfortable temperature.
  • Maintain the physical plant and agency property in a clean and safe manner and communicate any issues to management.
  • Provide all other duties as needed or requested to ensure that the individuals are safe, healthy, and comfortable.
  • Comply with OPWDD regulations and agency procedures.
  • Obtain and maintain SCIP-R certification; attend all mandatory training.
  • Obtain and maintain AMAP certification and submit a copy of the certification to each program in which you administer medication.
  • Attend all staff meetings, including in\-services and training.
  • Demonstrate competency in all required training areas.

Background Clearance

Acceptable background clearance check through the State Central Register and compliance with health regulations for physical examination and Mantoux.

Salary
  • Salary ranges from $19.00 – $20.00
  • DSPs with valid NYS Driver's License: $19.50
  • DSPs with valid NYS Driver's License & completed all mandatory trainings (e.g., CPR, SCIP, AMAP, etc.): $20.00
