Job summary

A community service organization is seeking an On-Call Life Skills and Educational Counseling position in Philadelphia. Responsibilities include supporting client safety, managing food preparation and inventory, assisting with medication and health management, and participating in educational activities. Ideal candidates will have strong collaboration skills and a commitment to maintaining safety and compliance during all shifts, which are required to be awake. Applicants must be ready to fulfill several scheduled trainings as well.

Qualifications

  • Ability to model and assist with meal planning and food inventory.
  • Strong skills in maintaining proper safety techniques for managing hazardous materials.
  • Experience in supporting client needs and proper food handling.

Responsibilities

  • Assist in maintaining program tasks and monitoring household chores.
  • Model and supervise mealtime organization.
  • Support nurses in managing client prescriptions.
  • Participate in team support meetings and document significant contacts.
  • Attend mandatory training and conduct fire drills.
  • Model and assist with meal planning and food inventory.
  • Support the creation of a grocery-shopping list and shopping as assigned by administrative staff.
  • Support and assist with monitoring of meal preparation and the implementation of the Nutrition Program.
  • Monitor and supervise mealtime organization.
  • Monitor the food, household needs, goods, and program meal budget.
  • Support and monitor client food and financial contributions to the program.
  • Manage and support clients with proper food handling and skills.
  • Monitor and manage proper storage and safety of all household goods.
  • Support and collaborate with nurses to assist clients with securing prescriptions when needed.
  • Support life skills challenges and ensure appointments are met; process challenges as needed.
  • Maintain proper safety techniques when managing hazardous materials.
  • Collect and document urine samples as assigned.
  • Retrieve and review medication upon delivery and during drop-offs when needed.
  • Monitor, review, and collaborate with nursing staff on client self-delivery of medication to the program.
  • Staff collaboration on client-individualized treatment needs and schedule changes.
  • Assist in conflict resolution.
  • Provide crisis intervention.
  • Participate in program milieu activities.
  • Conduct and facilitate, attend and/or assist with educational groups and meetings (e.g., AA, NA, Community Meeting) as assigned by Director.
  • Actively support socialization and recreational activities during programming.
  • Submit Sleep Wake Diary into AVATAR System.
  • Create and organize client charts using a table of contents.
  • Scan documents into the AVATAR System.
  • Document educational groups and other significant contacts.
  • Conduct, facilitate, and document fire drills.
  • Participate in Team Support Meetings and supervision as assigned by Director and Assistant Director.
  • Read daily logs and document in logs at shift start and end.
  • Support the safety of all participants.
  • Complete intake packets and inventory upon admissions and discharges as needed.
  • Escort clients to appointments and shopping as designed within the program schedule or according to the treatment plan.
  • Document daily in Sign-In logs, Refrigerator logs, Shift logs, and monitor client movement.
  • Ensure the program Accountability Schedule is followed; perform any other tasks assigned by the Director to ensure continuity of care.

Job description

ON CALL - INCLUDING WEEKEND SHIFTS (MUST WORK 3-4 SHIFTS PER MONTH)

Life Skills and Educational Counseling
  • Assist in maintaining the physical plant by monitoring and inspecting household chores.
  • Model and assist with completing proper program household tasks.
  • Maintain the program logbooks daily.
  • Implement the daily program schedule by ensuring tasks are completed as assigned by Administrative Staff.
  • Support client safety and apply counseling provisions when needed.
Food and Household Goods
  • Model and assist with meal planning and food inventory.
  • Support the creation of a grocery-shopping list and shopping as assigned by administrative staff.
  • Support and assist with monitoring of meal preparation and the implementation of the Nutrition Program.
  • Monitor and supervise mealtime organization.

Training

Attend scheduled/mandatory trainings as outlined in CBH, DDAP, and the employee’s individual training plans. All shifts are awake shifts.
