Job summary

Seven Hills Community Services is seeking a Direct Support Professional II to provide daily care and support in a residential setting. You will help individuals build independence by teaching life skills and by following ISP and Behavior Support Plans.

You’ll assist with personal care and medication administration, accompany to appointments, and maintain accurate documentation while mentoring other staff. A driver’s license and 18+ age are required.

Qualifications

• Strong communication, teamwork, and problem-solving abilities.
• Active US driver’s license is required.
• Must be at least 18 years old.

Responsibilities

• Provide daily support and care to individuals in a residential setting.
• Help individuals build independence by teaching life skills at home and in the community.
• Follow and implement Individual Support Plans (ISP) and Behavior Support Plans.
• Assist with personal care, medication administration, and health-related needs.
• Take a leadership role by training and mentoring other staff.
• Maintain accurate documentation and data collection to track progress and outcomes.

Cape-based programs offer a $1/hour differential. Martha’s Vineyard programs offer a $4/hour differential, with the exception of the Franklin Street program, which offers $3/hour.
