Job summary

365 Health Services is seeking a Direct Support Professional to provide personal care and support in Downingtown and surrounding Chester County communities. You will assist individuals with daily living, promote ISP-driven goals, and help people live independently within their community.

The role requires a valid driver's license and reliable vehicle, and offers in-person work with pay of $18.00 - $20.00 per hour.

Qualifications

• Assist individuals with daily living and community integration skills using Person-Centered Practices.
• Support ISP goals through training and engagement.
• Provide personal care, including bathing, grooming, dressing, toileting, and hygiene.
• Assist with home-living tasks such as meal preparation, laundry, light housekeeping, and shopping.
• Monitor and report changes in condition or needs to the Executive Director or designee.
• Complete documentation and daily progress notes accurately and on time.
• Maintain safety standards, infection control practices, and proper equipment use.
• Communicate effectively with team members and maintain confidentiality.
• Attend required training and compliance programs.
• Perform other duties as assigned.
