Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) in Cassville, PA. This field-based role requires hands-on, person-centered support to individuals with IDD in home and community settings, with focus on daily living activities, health monitoring, and community inclusion.

The DSP will implement ISP/IPP plans under supervision, maintain documentation, and participate in trainings and team meetings to ensure high-quality care compliant with regulations.

Qualifications

• High school diploma or equivalent required.
• Experience working with IDD/IDD clients is preferred.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Provide person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meals, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and wellbeing.
• Maintain accurate, timely documentation and communication with the team.
• Support transportation to appointments and community activities.
