Provides support for individuals and/or groups of persons with disabilities in the community. Assists the person(s) receiving services in identifying and communicating their requests and needs for services and supports. Implements services and supports to address the person’s goals and desired outcomes as identified through each person’s Centered Plan. Seek to understand the individuals in the context of their personal history, their social and family networks, and their hopes and dreams for the future. Relies on direction and supervision to accomplish the job.
Pay: $15.68/hr
Schedule: Full‑time, 1st shift, Monday‑Friday
This position has no supervisory responsibilities.
