Friendship Community Care is seeking a Direct Support Professional to join an interdisciplinary team in the home and community setting. You will assist clients with daily living tasks, provide transportation, and document progress toward care plans.

Adherence to HIPAA and confidentiality policies is required. Qualifications include a high school diploma or GED, or 2+ years’ experience with developmentally disabled individuals, plus a valid driver’s license.

  • Organize and carry out recreational activities for clients.
  • Maintain confidentiality per HIPAA and policy guidelines.

Overview

The Direct Support Professional works on an interdisciplinary team that delivers high-quality service and provides appropriate activities in a safe and healthy environment within the home and community setting.

Responsibilities
  • Assist Clients in basic living skills and daily personal tasks, including showers, dressing, housekeeping, grocery shopping, meal preparation/planning, and financial budgeting.
  • Provide transportation to and from activities and appointments as needed; a valid Arkansas Driver's License is required.
  • Maintain communication via appropriate channels (facility logbook, staff meetings, etc.) regarding clients’ needs and progress toward their plan of care by the end of shift.
  • Accurately complete documentation required by FCC, including Behavioral Reports, Incident Reports, Complaint Forms, Personal Care Work Sheets, Requisitions, Maintenance Forms, Progress Reports, Monthly Attendance Logs, and other required documents.
  • Follow and maintain records for FTA vehicles and mileage/billing forms in compliance with Medicaid guidelines.
  • Report incidents and/or emergencies to supervisors or delegated authorities immediately; this is a safety-sensitive position.
  • Assist in formulating, managing, revising, and following through on the client’s Plan of Care to ensure health, safety, and security are prioritized.
  • Follow medication management guidelines and assist in monitoring and documenting these procedures.
  • Organize and carry out recreational activities for clients.
  • Model professional behavior, maintain FCC dress code, and demonstrate appropriate appearance and actions.
  • Treat clients and their families with dignity and respect; represent the organization’s mission in all actions.
  • Assist in maintaining FCC facilities and grounds, ensuring cleanliness and orderly condition.
  • Assist in vehicle maintenance and cleanliness; support regular preventive maintenance as needed.
  • Attend required training and pursue continuing education.
  • Provide proof of liability insurance and keep it current.
  • May be required to undergo periodic driving tests and demonstrate proficiency in operating a motor vehicle, including passenger vans; may drive personal vehicle as needed with adequate liability coverage.
  • This is a safety-sensitive position.

  • Adhere to the Code of Conduct and Compliance Investigation Procedures as outlined in new employee training.
  • Maintain progress notes on prescribed forms daily or as needed.
  • Maintain consumer records weekly, monthly, quarterly, and yearly; file and update information as required by funding sources.

  • Education and/or Experience: High school diploma or GED; or two+ years of experience working with developmentally disabled individuals. Must also have a valid driver’s license.
  • Language Skills: Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to present information effectively in one-on-one and small group settings.
  • Mathematical/Computer Skills: Ability to perform basic arithmetic and interpret data (rates, ratios, percentages) and work with simple graphs.
  • Reasoning Ability: Ability to apply common sense understanding to carry out instructions and interpret various forms of directions.
  • Physical Demands: Regular use of hands, standing, walking, reaching, sitting, climbing/balancing, stooping, kneeling, crouching, crawling, and occasional lifting up to 100 pounds. Vision requirements include close, color, depth perception, and focus adjustment.
