• Valid driver's license and good driving record required.

Responsibilities

• Provide or coach individuals on personal care needs with limited supervision; administer medications as prescribed and monitor conditions.
• Teach and support skills to enable productive lives; help develop and modify plans to promote independent choices.
• Organize and participate in care coordination meetings; complete documentation for direct services to ensure timely, accurate record-keeping.
• Transport individuals to appointments, activities, and work.
