Job summary

Supportive Living Services is seeking a compassionate team member to assist individuals in enhancing independence and community involvement in Erie and surrounding areas.

The role emphasizes positive interaction, daily living support, and collaboration with the Program Director. Training is provided, and the schedule offers day/evening options with mileage reimbursement for personal vehicle use.

Qualifications

  • Must be 18 or older.
  • High School diploma or equivalent.
  • Valid driver's license and insurance.
  • Able to provide transportation utilizing personal vehicle (Generous mileage reimbursement included).
  • Proof of vehicle inspection.
  • Registration card and vehicle insurance card.

Responsibilities

  • Relate, interact, and communicate positively with the individuals.
  • Support, and assist the individuals in enhancing skills that will further their independence, increase their community involvement, and improve their quality of life.
  • Participate with the Program Director in the completion of individual assessments.
  • Maintain records of individual’s progress in specific program areas.
  • Assist and/or instruct individuals in activities of daily living (meal prep, housekeeping, shopping, laundry) and basic self-care.
  • Document each encounter with the individual as it relates to their ISP outcomes.
  • Motivate and assist individuals in all ISP-designated outcomes.
  • Display a positive, flexible, cooperative attitude toward supervisors, coworkers and the general public.
  • Assist individuals with medical appointments.
  • Report pertinent medical information on continuity forms to relevant parties.

Supportive Living Services believes that all people should live the best life possible. The mission of the SLS Autism & Bridge Program is to aid, assist, guide, coach, and mentor individuals in any area where we can help. Staff receive the gratification to focus on clients goals and help them grow into fulfilled, independent members of the community to the greatest extent possible.

SLS provides community services to individuals living in their own homes in Erie, NorthEast,Edinboro, Girard, and Albion.

In this position you not only have an opportunity to join a supportive team of passionate individuals, but also to learn the skills and gain experience!

Starting rate of $18.00 per hour

Paid training

Flexible work schedule (Days and evenings available)

401 K available for part time and full time staff

Requirements:
  • Must be 18 or older

Essential Duties and Responsibilities:
  • Relate, interact, and communicate positively with the individuals.
  • Support, and assist the individuals in enhancing skills that will further their independence, increase their community involvement, and improve their quality of life.
  • Participate with the Program Director in the completion of individual assessments.
  • Maintain records of individual’s progress in specific program areas.
  • Assist and or instruct individuals in activities of daily living (meal prep, house cleaning, grocery shopping, laundry and general maintenance) self-care skills (bathing, dressing, personal hygiene); safety and prevocational skills when appropriate.
  • Document each encounter with the individual as it relates to their lSP outcomes.
  • Motivate and assist the individuals in all areas designated in the ISP as outcomes.
  • Display a positive, flexible, and cooperative attitude toward supervisors, coworkers and the general public.
  • Assist individuals with medical appointments.
  • Report or communicate all pertinent medical information on a continuity of care form to all appropriate persons (i.e. Program Supervisor(s), Medical Department, Supports Coordinator and Family Members).
  • Assist individuals in development of skills to independently advocate for themselves. Procure necessary services as needed.
  • Meet with Program Supervisor(s) as needed to clarify job responsibilities, discuss individual needs.
  • Attend trainings and in-services as required.

This position requires you to frequently stand, walk, climb stairs, squat, and bend. Must routinely lift 50 lbs or more.
