General Function

The position involves providing support to individuals with disabilities, empowering them to lead self‑directed lives, and providing them with opportunities to develop and maintain skills that enhance the quality of their lives. Our mission is to ensure that supported persons are happy, healthy and safe in an environment where they can enjoy life to the fullest extent possible.

Essential Functions

• Supports the person with personal care and daily routines, and helps them achieve outcomes from the Individual Support Plan.
• Understands and practices Support Solutions philosophy and values in interactions with the person and community members, using language and actions that respect the worth and dignity of people at all times, connecting the person to preferred activities and experiences of their choice, and ensuring empowerment to make decisions and lead a self‑directed life.
• Helps the person become integrated with the community by providing person‑centered supports that assist in seeking their dreams, establishing significant relationships, and connecting to their communities.
• Assures safe transport and accompanies the person to events, job sites, and medical appointments using Support Solutions vehicles, personal vehicles, or public transportation. Also secures alternate transportation if an unpaid supporter transports a person who presents a safety concern.
• Ensures proper use of the person’s and agency’s resources including personal funds, vehicles, buildings, utilities, telephones, money, equipment, food, and supplies.
• Attends all training meetings assigned by Support Solutions.
• Prepares and provides meals based on scheduled menus.
• Performs daily activities or teaches the person to perform activities of daily living skills and other personal skills that work toward independence.
• Consistently documents improvement toward meeting outcomes.
• Utilizes electronic records systems, including completing daily shift notes as required by funding entities.
• Follows Customer Service Standards when interacting and communicating with persons supported, their families, conservators, vendors, ISCs, case managers, and others.
• Oversees storage and administration of medications.
• Assists the person with personal grooming as desired, supporting independence whenever possible.
• Assists the person with keeping the house clean and doing laundry as desired.
• Protects the confidentiality of the person supported.

Qualifications

Minimum Requirements

• Must be 18 years of age and able to read and write effectively.
• High school diploma or GED.
• Valid driver’s license with an acceptable driving record throughout employment.
• Must meet state and local requirements regarding criminal background check, sexual offender check, abuse registry, drug and felony offender listings.
• Must meet all training requirements and keep training current.

Medical Requirements

Employee must provide any medical records to comply with regulations required by governing entities and Support Solutions policy. Examples include TB skin tests, HEP B vaccinations, and flu vaccinations.

Physical Requirements

The physical demands described here are representative of those that must be met by the employee to successfully perform the essential functions of this job. This position may require additional hours over 40 per week and may be primarily in a home environment, but also requires presence with supported individuals in community settings, work settings, outdoor activities, etc.

Body positioning: regular standing and walking for prolonged periods; occasional sitting for short or long periods; occasional stooping, kneeling, or crouching; and reaching with hands and arms.

Driving/travel for short periods to transport supported persons to appointments, community outings, meetings, and trainings.

Physical strength: ability to perform heavy lifting over 50 lbs. to assist transfers, repositioning in bed, and other tasks.

Full range of body movements and sensory perception required for safe client care (vision, hearing, touch, smell, taste).

Licenses / Certificates

• Valid state‑issued driver’s license.
• Valid medication certification throughout tenure.
• Valid CPR/First Aid certification throughout tenure.
• Valid Crisis Prevention Institute (CPI) training throughout tenure, if required.

Agency Insurance Coverage for Vehicles

Agency insurance covers vehicles operated in the course of work at specific locations; employees working in homes where the agency does not provide a vehicle must present valid automobile insurance prior to transfer to any such home. The agency reserves the right to request proof of insurance regardless of site location or availability of an agency vehicle.
