Job summary

The Association for Individual Development is seeking a Direct Support Professional – 24HC – Program Support for Elgin, IL. Responsibilities include assisting clients with daily living, following individualized plans, and coordinating with staff to ensure safety and dignity.

Positions are full-time, first shift, with hours between 7a-6p. Requires High School diploma or GED and experience with disabilities; training and license requirements apply.

Qualifications

• High School diploma or GED required.
• Minimum one year experience with people with intellectual or developmental disabilities.
• Three years’ DSP experience preferred.
• CNA or Autism certificate desired.
• Must lift or move up to 50 pounds.
• Pass S-TOFHLA literacy test.
• Complete DSP and Medication Administration Training including CPR, First Aid, Infection Control, OIG Rule 50 and CPI.
• Must have valid Illinois driver’s license and transport clients when needed.

Responsibilities

Schedule

Full time – 40 hours, First shift. Flexibility to work hours between 7a-6p.

Responsibilities

• Maintain a team approach in working with clients, staff, and outside agencies.
• Implement specific assigned programs from the Individual Plan or Implementation Strategy, and document per program procedures.
• Follow all agency and program rules and regulations; report concerns or issues to immediate supervisor without delay.
• Plan and prepare meals by following menus and recipes written by dietician with client input and assistance; adhere to client diet requirements.
• Provide self‑care assistance to clients including toileting, changing garments, bathing, grooming, shaving, cleaning glasses and adaptive equipment.
• Obtain medication administration authorization at a minimum of one (1) location and maintain it by attending annual training and following all policies.
• Ensure safe conditions inside and outside the home; maintain housekeeping, report safety concerns, enter requests for non‑urgent repairs, ensure bedrooms and décor meet regulatory standards, perform safety drills and monitor tasks.
• Maintain a clean, sanitary environment in all assigned buildings and vehicles daily.
• Be alert to emergency situations and take appropriate action per program and agency policies.
• Transport clients in personal or agency vehicle for work, shopping, appointments, social events, etc.
• Know client medical conditions; monitor and relay information to and from medical appointments.
• Attend required meetings, shifts, and trainings.
• Assist with orientation/training of new staff.
• Participate in planning and implementation community and in‑house activities.
• Conduct monthly client input meetings, facilitate Home and Community Based Settings Rules training activities, and other client meetings as assigned, documenting participation.
• May be responsible for independent building coverage, including safety and supervision of all clients, per program.
• Work scheduled hours at various locations as required by client and program needs.
• Complete required training on or prior to due date to meet licensing and program requirements.
• Monitor, schedule, and complete routine and follow‑up medical appointments for assigned clients.
• Monitor and maintain Public Entitlement benefit status – attend appointments, complete paperwork, attend phone interviews.
• Assist with staff training and/or retraining on client plans; lead by example while working alongside DSPs in homes.

• Evening, weekend, overnight, and holiday hours required; DSP schedules are subject to change.
• Must attend scheduled in‑service trainings.

Physical Requirements

• Climb stairs daily.
• No medical restrictions prohibiting implementation of a lift/transfer or behavioral programs.
• Push, pull, reach, stoop/bend over and use step stool daily.
• Perform daily housekeeping tasks and assist clients in independent living skills.
• Assist clients in situations such as getting in and out of van, bathtub, shower, or bed regularly.
• Implement behavior support strategies and respond to emergency behavioral issues.
• Requires significant walking and standing daily.

Equipment

• Computer and software: Microsoft Word, Outlook, Excel, Teams; Zoom platform and application; Internet browser and Microsoft SharePoint platform.
• Household appliances: stove, microwave, washer/dryer, dishwasher, countertop appliances.
• Alarm systems for monitoring fire, weather, security, etc.
• Telephone and voicemail.
• Office equipment: printer/scanner, copy machine, calculator, shredder, etc.
• Adaptive equipment per individual strategies: Hoyer lift, shower chairs or benches, walkers, wheelchairs, etc.
• Agency vehicles with and without wheelchair lifts.
