Job summary

Cap City Supported Living in North College Hill, OH is seeking compassionate caregivers to provide direct care tailored to each client's ISP, including daily living tasks and safety support.

You will assist with activities, transportation, medical appointments, medication support, and accurate documentation while complying with HIPAA and agency policies. Weekend shifts offer extra pay per hour.

Qualifications

• High School Diploma or equivalent required.
• Valid Driver's License (no more than 6 points)
• Background Check (FBI and/or BCI) as per ORC.
• CPR and First Aid Training required.
• 8hr DODD Training and Medication Administration Training required.

Responsibilities

• Provide direct care per client ISP.
• Assist with daily tasks: housekeeping, meals, shopping, personal care.
• Support safety: drills, emergencies, and de-escalation as needed.
• Assist with day-to-day finances.
• Facilitate access to community activities and relationships.
• Schedule and attend medical appointments and manage medications.
• Provide safe transportation to appointments and activities.
• Complete documentation per agency, county, and client requirements.
• Advocate for clients' rights and abide by agency policies.

Cap City Supported Living & Home Care Services

Cap City Supported Living provides support services geared toward young adults and adults who live with developmental disabilities. Our clients benefit from socialization and community- bases programs, and Home Personal Care.

Essential Job Functions:

• Provide direct care to client's based upon their needs as well as the client's Individual Services Plan (ISP).
• Coach/assist client with accomplishing routine tasks, housekeeping, meal prep, grocery shopping, and personal care.
• Coach/assist clients in the area of safety by performing routine fire and tornado drills, recognizing emergencies as well as responding as needed.
• Coach/assist client with day to day finances.
• Assist client with access to available community activities, exercise, and maintaining relationships.
• Assist clients with monitoring medical needs including scheduling, attending medical appointments, filling prescriptions, and self or delegated administration of medications or health related activities.
• Provides or facilitates safe transportation in regards to social, medical and general needs.
• Attend all meetings in regards to clients, employee requirements, and or agency needs.
• Complete all documentation accurately and services according to the agency, county and client requirements.
• Complies with OSHA, HIPAA, Rights of Clients (5123.62 ORC), Clients Options Waiver/HPC (5123:2-13-04-OAC)
• Advocating to protect and advance the rights of clients
• Embrace and embody the mission, vision and policies and procedures of Cap City.
• Communicate with supervisors, peers on any medical, health and safety needs.
• Other duties as assigned.

Essential Qualifications:

• High School Diploma or equivalent.
• Valid Driver's License (no more than 6 points)
• Current Proof of auto insurance (state minimum)
• 18 years or older
• FBI and or BCI Background Check (5123:2-1-05.1 ORC)
• CPR and First Aid Training (5123:2-2-.1 ORC)
• 8hr DODD Training
• Medication Administration Training
• Any other trainings deemed necessary by Cap City Home Care Services

Working Conditions:

• Work is performed primarily in residential setting.

• Requires mobility and physical activity: Having adequate range of motion and mobility to work in an office, residentials, or outdoor environment including standing and walking (even and uneven surfaces), sitting for extended period of time, bending, twisting, reaching, balancing, and occasionally lifting and carrying up to 30 pounds.
• Requires the ability to defend oneself and clients in physically aggressive situations through the use of approved descalation techniques.
• Requires communication, expressing or exchanging ideas by means of spoken and written word. Talking is required to share information regarding clients.
• Requires hearing: Hearing is required to receive and communicate detailed information thought oral communication and ensure ability to supervise clients.
• Requires seeing: Clarity of vision at 20 inches or less and at a distance to complete paperwork, observe and supervise client.

• Requires the ability to reason and be empathetic to clients and teammates.
• Requires the ability to cooperate with clients, teammates, supervisors and outside supports for clients to exchange ideas, information and instruction.
