Job Summary
Responsible for creating, leading, and supporting assigned participants in their daily programming in the areas of academic, social, life and leisure skills as well as recreational activities. Assists in the development and implementation of Personal Centered Individual Service Plans (ICP) including social, community inclusion, communication, behavior, and skill development. Ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, and CARF as well as other funding sources and company policy/procedure.
Primary Duties
- Provide active engagement and support for all individuals assigned, including community integration/inclusion, training, leisure, communication, mobility, behavior, personal care (feeding, etc.), bus loading/unloading, medication, safety and risk management.
- Provide input and implement Person Centered Plans (PCP) outcomes, support activities, and protocols as written to ensure a safe and healthy environment and proper documentation and progress. Maintain accurate information for the PCP, quarterly reports and daily notes.
- Maintain a safe, clean, and orderly environment. Immediately report any unsafe conditions or medical concerns to the Program Manager (or designee). Follow established written protocols and plans. Report any allegations of abuse, neglect, or exploitation according to ServiceSource policy/procedures. Complete incident reports as required.
- Ensure accuracy of records and documentation per the PCP, regulatory and company standards. Inform the Program Manager of missing or inaccurate information immediately.
- Communicate with family members, guardians, authorized representatives, residential providers and Support Coordinators regarding participant's illness, incidents, progress, etc. as directed by the Program Manager. Keep team members apprised of any concerns as needed.
- Perform related duties and responsibilities as assigned or requested.
Additional Duties and Responsibilities
- Maintain participant records, daily notes, quarterly reports, etc. per Medicaid, Licensure and CARF standards.
- Adhere to health and safety regulations.
- Participate and/or provide input for the PCP process (plan, quarterlies, daily notes) and work with the team to implement, monitor and review the individual’s written plan.
- Utilize appropriate behavior intervention techniques, proper lifting, feeding, and transferring techniques as written.
- Complete and pass the Orientation to Development Disabilities (ODD) test and competencies.
- Participate in department, participant, and staff committee meetings.
- Participate in trainings as required by funding sources and the corporation.
Qualifications: Education, Experience and Certification(s)
- Required: A High School diploma (or GED) and at least one year of experience working with adults with developmental disabilities or in a related field.
- Preferred: High School diploma (or GED) plus college‑level courses in the human services field and experience working with adults with developmental disabilities, or a bachelor's degree in the human services field and experience working with individuals with developmental disabilities.

- Knowledge of communication and its resources.
- Physical ability to support individuals with personal care (feeding, toileting, transferring, etc.).
- Drivers whose duties involve transporting participants must possess a current and valid driver's license and be covered under ServiceSource's driver's insurance. Must pass the online driving training if eligible to drive.
- Ability to pass a criminal background check and drug screening as required.
- Working knowledge of Microsoft Office software and other basic computer programs.
Physical Demands
While performing the duties of this job, the employee is regularly required to stand; reach with hands and arms; and talk or hear. The employee is occasionally required to walk and sit. The employee may occasionally lift or move products and supplies, up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, depth perception and the ability to adjust focus.
