Job summary

Lutheran Social Services of Illinois is seeking a Direct Support Person to assist individuals with self-care, hygiene, meals, housekeeping, socialization and recreation. This full-time role includes documenting progress and servicing plans while supporting transportation needs.

Qualifications

• High school diploma or equivalent.
• Background check clearance required.
• CPR and First Aid basics required.
• Must be 18 years of age or older.

• Valid driver’s license in good standing and reliable transportation.

Responsibilities

• Monitor, implement, and document individual personal goals and behavior programs.
• Participate in development of the Service Plan via daily log, programming suggestions, and staffing.
• Provide transportation to the person(s) served as needed.
• Maintain regular and consistent work attendance.
• Demonstrate professional, respectful conduct toward clients, families, staff, and partners.

Essential Functions

Position Qualifications
