Job summary

PACE Southeast Michigan is seeking a Direct Home Care Worker to provide in-home and community-based support, following individualized care plans and maintaining compassionate, respectful care for participants.

Responsibilities include personal care, meal prep, documentation, and coordination with nurses. Requires a high school diploma or GED, relevant experience or training, and ability to work with diverse populations within CMS guidelines.

Qualifications

  • Minimum high school diploma or GED; two years of relevant experience or completion of a formal training program.
  • Ability to establish and maintain interpersonal relationships with participants and coworkers.
  • Organize and maintain flexible scheduling to coordinate responsibilities.
  • Work sensitively with diverse ethnic and cultural backgrounds.
  • Perform duties amid interruptions and follow CMS-approved competencies.
  • Be medically cleared for communicable diseases and up-to-date immunizations.

Responsibilities

  • Assist in providing and maintaining comfort for frail elders, participants, and families.
  • Follow the participant’s Care Plan and relevant guidelines as determined by an RNCM.
  • Perform personal care, meal preparation, toileting, simple treatments, shopping, reminders, laundry and homemaking tasks, transportation, and appointment support.
  • Keep accurate documentation and report progress to the RNCM or clinic nurse.
  • Report customer service issues, schedule deviations, and emergencies promptly to supervisor.
  • Work within the established scope of practice and respond to emergencies per policy.
  • Attend in-service trainings and screenings (infection control, TB testing, safety, BLS).

Summary

The Direct Home Care Worker provides supportive services to participants in the home, community setting, day health center, during transport, and at appointments.

Responsibilities
  • Assist in providing and maintaining optimal physical and emotional comfort for frail elders, participants, and families.
  • Follow the participant’s Care Plan and relevant Care Guidelines as determined by an RNCM.
  • Perform personal care assistance, meal preparation/feeding, toileting/incontinence management, simple treatments, shopping, medication reminders, laundry services, homemaking chores, transportation, and off‑site appointment support.
  • Keep accurate and timely documentation of all services provided and report progress to the RNCM or clinic nurse.
  • Immediately report customer service complaints, deviations from schedules, and emergencies to the supervisor or manager.
  • Work only within the established scope of practice and respond to emergency situations per policy.
  • Participate in annual required in‑service training and screenings (infection control, TB testing, safety training, BLS training).
Qualifications
  • Minimum high school diploma or G.E.D.; at least two (2) years of relevant experience or completion of a formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.).
  • Ability to establish and maintain interpersonal and interdepartmental relationships and respect the well‑being of participants and coworkers.
  • Organize and maintain flexible scheduling to coordinate job responsibilities.
  • Work sensitively with individuals of diverse ethnic and cultural backgrounds, including those with dementia or developmental disabilities.
  • Perform duties in an environment with frequent interruptions.
  • Meet CMS‑approved standardized competencies before working independently.
  • Be medically cleared for communicable diseases and have all immunizations up to date before providing direct participant contact.
Working Conditions
  • Driving required within the PACE Southeast Michigan catchment area; reliable transportation and a current valid driver’s license are mandatory.
  • Exposure to communicable diseases, cleaning solutions, and medications is possible.
  • Work conditions vary and may involve frequent interruptions and exposure to hazardous materials.
