Centra Healthcare Solutions, Inc. is hiring a Physical Therapist (PT) for a SNF/LTC assignment in California. This direct hire/perm role references a days schedule and carries responsibility to align with the organization’s vision, values, and goals.

The PT will develop plans of care, deliver skilled PT interventions per physician orders, assess and adjust care, and collaborate with multi-disciplinary teams to meet patient goals. Licensure and CPR certification are required.

Qualifications

• Graduate from an accredited school.
• Current state license as a Registered Physical Therapist (PT) or actively pursuing licensure.
• Current CPR certification.

Responsabilités

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
