Responsibilities

• Support community integration and transportation as needed.
• Monitor and supervise individuals at all times during service, including transport and community activities.
• Participate in Communicare Treatment Team meetings and perform other duties as assigned.

• Services provided will include:
• Assistance and acquisition, retention, or improvement in social self-help, socialization, and other adaptive skills that enhance social development and skills in performing activities of daily living (i.e. money management, medication management)
• Meaningful day activities for the Individual to include the acquisition and maintenance of skills, building positive social behavior and interpersonal competence, greater independence and personal choice
• Community integration activities
• Maintain responsibility for monitoring and supervising individuals at all times during service provision, whether in a supervised living home, during transporting, or in the community.
• Serve as a member of the Communicare Treatment Team and participate in team meetings as required
• Perform other duties as assigned.
• Work under the supervision of the Supervised Living Supervisor or other supervisor as designated.
• Transport Clients to various activities, appointments, etc.
