Job summary

The Center for Human Development (CHD) seeks Direct Care Staff to join our Community Transitional Living Program (CTLP) in Hartford, CT. Our residential staff establish life-changing relationships with our participants and support their independence, rehabilitation, and recovery.

CTLP is a community-based transitional living environment for up to 19 adults with severe mental health diagnoses, with 24/7 staff monitoring and support.

Qualifications

• High school diploma or GED or equivalent.
• Valid driver's license required.
• Registered and insured vehicle available for work purposes.
• Approved criminal background and driving check.

Responsibilities

• Daily activities include providing direct services to program clients in a group home setting in the community.
• Support clients' activities of daily living and monitor self-administration of medication.
• Advocate for individuals in various community settings and help optimize independence, rehabilitation and recovery.
• Develop professional, supportive relationships and provide person-centered guidance.
• Desire to work with individuals with mental illness and/or substance use histories.

The Center for Human Development, (CHD) is seeking Direct Care Staff to join our team at our Community Transitional Living Program (CTLP) in Hartford, CT. Our residential direct care staff establish life-changing relationships with our participants and provide opportunities to optimize their independence, rehabilitation, and recovery.

CHD’s CTLP program is a community-based transitional independent living environment for up to 19 eligible adults with severe and persistent mental health diagnoses that will benefit from temporary residential support. Residents benefit from 24/7 staff monitoring and support while living in their own apartments within a single building, and working to achieve their goals, including discharging to even more independent living. Residents receive support in ADL’s, skill building, life skills, social/recreational activities, self-care, household tasks, education/employment, transportation, finances, and much more.

Your role as a Direct Care Staff:

The Direct Care Staff works within the Agency’s team model, establishing open communication, support, and accountability among co-workers. Providing a supportive residential environment for individuals who are challenged by mental health and substance abuse histories in a community-based group home. Our goal is to provide an environment that encourages program participants to grow and to live as independently as possible. Some additional responsibilities include, but are not limited to:

• Daily activities include providing direct services to program clients living in a group home setting in the community
• Supporting clients' activities of daily living and monitoring clients' self-administer medication
• Advocating for individuals' within various community settings, and providing opportunities to optimize their independence, rehabilitation and recovery
• Develop professional and supportive relationships with clients and provide informal counseling and guidance with a person-centered focus
• The ideal candidates will have a desire to work with members of our community who have mental illness and substance abuse struggles or both

SCHEDULES:

Full-time

2nd shift, 2:00pm-10:00pm. Days on: Tuesday, Wednesday, Thursday, Friday, Saturday. Days off: Sunday, Monday.

Part-time

2nd shift, 2:00pm-10:00pm. Days on: Sunday, Monday. Days off: Tuesday, Wednesday, Thursday, Friday, Saturday.
