CORA Physical Therapy seeks a licensed Physical Therapist to join as a Digital Health Specialist in a hybrid role blending in-clinic care and remote services. This full-time position supports virtual care, remote monitoring, and telehealth as part of our expanding digital health program.

You will develop personalized care plans, document progress, and collaborate with in-clinic providers to deliver high-quality outcomes. A CAPTE-approved PT program degree and licensure are required.

Qualifications

• Licensed PT in any compact state and willing to obtain licensure in other states.
• Diploma from a CAPTE-approved Physical Therapy program.
• Digital/telehealth experience preferred.

Responsabilités

• Develop and deliver personalized plans of care for orthopedic and neurologic patients.
• Objectively measure patient outcomes using digital software.
• Document evaluations, treatments, re-evaluations, and discharge notes.
• Deliver Telehealth visits including evaluation, assessment, and treatment planning.
• Perform Remote Therapeutic Monitoring (RTM) services and remote care.
• Collaborate with clinic teams and Director of Digital Health to grow remote services.
• Provide regular reporting and data analysis of regional service metrics.

Physical Therapist - Digital Health Specialist - Hybrid Role (Full Time)

Ignite Your Career With CORA!This is a hybrid position working 3 days in person at our CORA clinic and two days working remotely as a Digital Health Specialist. The Digital Health Specialist (DHS) provides digital health services, including virtual care and remote therapeutic monitoring. The DHS participates in the development of the digital health program and serves as a liaison between patients and in-clinic treating providers. This is a

full-time position (40 hours/week, 5 days/week) . Must be licensed in any compact state. Must also be willing to obtain licensure in other states.

What You'll Do:

Make a powerful impact on your local community through inclusive physical therapy treatmentDevelop and deliver a personalized plan of care for your patients -- a diverse patient population with both orthopedic and neurologic diagnosesObjectively measure patient outcomes using cutting-edge softwareEfficiently document evaluations, treatments, re-evaluations, and discharge notesUses digital platform to deliver telehealth visits including evaluation, assessment, and treatment plan development.Performs Telehealth and Remote Therapeutic Monitoring (RTM) services including interactive communication and monitoring.Works closely and supports operations to deliver digital services and meet patient care and staffing needs.Communicates with the primary treating clinician providing revisions in treatment as appropriate.Serves as a liaison between providers and Director of Digital Health to optimize and grow remote services at CORA.Maintains regular reporting and data analysis of regional service metrics.Participates in program development and marketing efforts as directed by the Director of Digital Health.
