Qualifications

• Clinical navigator role basics: coordinating care and using digital tools.
• Experience or training in care coordination or population health management.
• Ability to educate patients on online portals and telehealth.
• Familiarity with privacy and HIPAA compliance.

Responsibilities

• Guide patients through Kaiser Permanente digital health and care navigation tools.
• Coordinate care plans between patients, providers, and support teams using IT platforms.
• Educate patients on using online portals, telehealth, and mobile applications.
• Monitor patient progress, identify barriers, and escalate when needed.
• Document encounters and maintain accurate electronic health records.
• Collaborate with product and IT teams to improve navigation workflows.
• Analyze usage data to recommend process and technology enhancements.
• Ensure compliance with privacy, security, and organizational policies.
