Job summary

Visiting Nurse Association in Massachusetts is seeking a per diem Dietitian to provide nutritional consultation to staff and patients within the VNA-Nutritional Counseling department. You will assess patients, develop care plans, and coordinate with physicians to ensure quality care.

The role emphasizes accurate documentation, ongoing staff education, and collaboration with Cape Cod Healthcare’s network to support patient nutrition services.

Qualifications

• Bachelor of Science degree in Dietetics from an accredited school.
• Completed internship or Master’s Degree.
• Current Mass. Dietitians/Nutritionist (LDN) license.
• Current registration with the American Dietetic Association.

Responsibilities

• Provides consultation to agency staff concerning nutritional problems/concerns of the Agency population.
• Provides in-service educational programs for the staff of the agency as required.
• Performs patient assessment and evaluation when requested by the physician or staff clinician.
• Develops a plan of care for each patient receiving nutritional consultation, and in cooperation with the patient’s physician.
• Performs complete and timely record keeping according to agency policy for documenting accountability of care given, for continuity of patient care, for planning and evaluating.
• Develops and updates a resource file for reference and nutrition education handouts.
• All other duties as assigned.

Education

Bachelor of Science degree in Dietetics

Completed appropriate internship program and/or Master’s Degree
