Responsibilities

  • Provide nutritional care to patients through nutrition screening, assessment, counseling, and metabolic monitoring.
  • Develop and implement evidence-based nutrition care plans tailored to patients.
  • Evaluate and monitor effectiveness of Medical Nutrition Therapy interventions and follow up.
  • Perform Nutrition Focused Physical Exams to determine malnutrition risk.
  • Assess patients for food security and provide resources as appropriate.
  • Collaborate with interdisciplinary care team to optimize patient care.
  • Educate patients and families on nutrition considering cultural needs and readiness to learn.
  • Attend interdisciplinary rounds and meetings as assigned.
  • Manage parenteral nutrition support for patients.
  • Participate in departmental Journal Clubs and education sessions.
  • Contribute to quality and productivity improvement projects.

Job Summary

The candidate is responsible for providing nutritional care to patients through nutrition screening, assessment, counseling, metabolic monitoring, and communication with interdisciplinary team members.

  • Complete initial nutrition assessments in accordance with departmental policies and guidelines
  • Develop and implement evidence-based nutrition care plans tailored to patients
  • Evaluate and monitor the effectiveness of Medical Nutrition Therapy interventions and follow up appropriately
  • Perform Nutrition Focused Physical Exams to determine risk or degree of malnutrition
  • Assess patients for food security and provide resources as appropriate
  • Work with the interdisciplinary care team to optimize patient care
  • Provide education to patients and families on nutrition recommendations while taking into consideration cultural needs and readiness to learn
  • Attend interdisciplinary rounds or meetings assigned
  • Manage parenteral nutrition support for patients

Qualifications
