Responsibilities

• Under supervision by the PACE Program Manager, plans and provides nutritional care for Siouxland PACE participants as a member of the Interdisciplinary Team. Assesses nutritional needs, develops and implements nutritional care plans, and evaluates and reports these results appropriately. Maintains professional competency and skills required for professional practice.
• Maintains a working knowledge of applicable PACE Federal, State, and local laws and regulations, the Compliance Accountability Program, Code of Ethics, as well as other policies and procedures in order to assure adherence in a manner that reflects honest, ethical, and professional behavior.
• Obtains and evaluates nutritional and medical history, laboratory data, test results, current medications, current diet orders and nutritional regimen to determine participant’s estimated nutritional needs.
• Recommends and implements nutrition interventions that assist in maintaining or improving nutritional-related medical diagnoses.
• Performs other duties as assigned.
