Job summary

Fresenius Medical Care is seeking an entry-level Medical Social Worker (MSW) to provide psychosocial services to dialysis patients, including home dialysis if applicable. You will work with the interdisciplinary team to address emotional, social, and behavioral aspects of chronic kidney disease, support adherence, and assist with transplant referrals.

The role involves patient assessment, care planning, education, and coordination with insurance and community resources; rate of pay ranges

Qualifications

• Masters in Social Work; state licensure required per applicable scope of practice.
• 0–2 years’ related experience as stated in the job description.

Responsibilities

• Assess patients' psychosocial status and collaborate on care planning to meet rehabilitation goals.
• Provide monitoring and interventions to support adjustment to dialysis and quality of life.
• Offer education to patients and families about psychosocial issues and transplant options.
• Coordinate referrals to community resources and vocational rehabilitation as needed.
• Assist with patient education on rights, grievances, and facility policies; support DNR discussions as appropriate.
• Participate in quality review meetings and interdisciplinary team huddles.
