Job summary

Avera St Luke’s Hospital in Washington seeks a Social Worker to plan, organize and implement social work services for medically and socially complex patients and families. You will work within a multidisciplinary team to ensure access to care and patient advocacy.

Responsibilities include interviewing patients/families, discharge planning, coordinating referrals, counseling, and maintaining EMR documentation. Requires MSW and state licensure; QMHP preferred; 1–3 years experience is expected.

Qualifications

• Master’s in Social Work from an accredited program.
• Active license in the state of practice Upon Hire.
• QMHP certification preferred; licensing board active Upon Hire.
• 1–3 years of relevant experience.

Responsibilities

• Interviews patient and family to assess home and social needs.
• Plan discharge and coordinate post-acute care as needed.
• Counsel patients and families and collaborate with the care team.
• Maintain EMR documentation and ensure quality care coordination.
