Responsibilities

• Performs initial and ongoing assessment of the patient.
• Communicates and documents assessment with the interdisciplinary team.
• Develops and documents plan of care with the interdisciplinary health team.
• Provides care, treatment and services in collaboration with the team.
• Evaluates patient responses to care and revises the plan as needed.
