Concerto Renal Services is seeking a Dialysis Nurse to join our team. You will supervise dialysis care, coordinate with physicians, and ensure high-quality patient outcomes in a sub-acute hemodialysis setting within a skilled nursing and/or long-term care facility.

The role emphasizes hands-on patient care, team leadership, and regulatory compliance, with a focus on education, documentation, and collaboration across the care team.

Responsibilities

• Provides direct hemodialysis care, performs assessments, administers medications, and educates patients on renal health and self-care.
• Delegates tasks to dialysis technicians, supervises performance, and ensures compliance with clinical standards and regulations.
• Monitors patient response to dialysis, adjusts treatment plans as needed, and documents interventions and outcomes.
• Coordinates care with physicians and healthcare team, reports critical labs, and ensures continuity of treatment.
• Maintains electronic medical records, completes care plans timely, and adheres to safety and regulatory protocols.
