Concerto Renal Services is seeking a Dialysis Nurse to supervise dialysis care within a skilled nursing or long-term care facility. You will oversee the unit, work hands-on with patients, and collaborate with the hemodialysis team to ensure compliant, high-quality care.

You will educate patients, monitor responses to dialysis, and support physicians in coordinating treatment while maintaining accurate documentation and safety protocols.

Responsibilities

• Provides direct hemodialysis care, assessments, medications, and patient education on renal health.
• Delegates tasks to dialysis technicians, supervising performance and ensuring standards.
• Monitors patient response, adjusts treatment plans, and documents outcomes.
• Coordinates with physicians and healthcare team to report labs and ensure continuity.
• Maintains accurate EMR, care plans, and follows safety protocols.
