Job summary

The County of Santa Clara is seeking an experienced Registered Nurse to assess, plan, implement and evaluate ESRD-related education and health services for a defined patient population.

Position requires day-one proficiency with CMS Conditions for Coverage, QAPI programs, and dialysis Plan of Care requirements, plus CA RN licensure and BSN. Travel between hospital/clinic sites may be required.

Qualifications

• Licensed Registered Nurse in California (BRN) and BSN required.
• Master’s Degree (MSN) desirable.
• Three years of clinical experience in the specialty area preferred.

Responsibilities

• Assess patients' clinical needs and staffing requirements for the ESRD specialty.
• Collaborate with physicians and health teams to plan and coordinate dialysis care.
• Educate staff and patients on Plan of Care, CMS 42 CFR 494, QAPI and quality metrics.
• Coordinate patient flow and maintain care continuity across sites.
