Concerto Renal Services is seeking a Dialysis Nurse to oversee hemodialysis care within a skilled nursing facility. The role involves supervising dialysis technicians, delivering direct patient care, and ensuring accurate documentation in line with regulatory standards.

You will collaborate with physicians and the healthcare team to coordinate treatment, monitor patient responses, and adjust plans as needed, while promoting patient education and safety.

Formación

• Supervise dialysis care in accordance with policy, procedure and training and in compliance with state and federal regulations.
• Coordinate care with physicians and the hemodialysis healthcare team to ensure continuity of treatment.
• Maintain accurate documentation of interventions and patient outcomes.

Responsabilidades

• Patient Care & Education: Provide direct hemodialysis care, assess patients, administer medications, and educate on renal health and self-care.
• Team Leadership & Supervision: Delegate tasks to dialysis technicians, supervise performance, ensure compliance with standards.
• Treatment Monitoring & Adjustment: Monitor patient response, adjust treatment plans as needed, document interventions.
• Collaboration & Communication: Work with physicians and healthcare team to report critical lab results and coordinate care.
• Documentation & Compliance: Maintain EMR records, complete care plans timely, follow safety and regulatory protocols.
