Job summary

Concerto Renal Services is seeking a Dialysis LPN to supervise dialysis care in accordance with policy, procedure and training and in compliance with state and federal regulations. You will supervise care provided by dialysis technicians and work hands-on with the team to ensure patient safety.

The ideal candidate is compassionate, an effective communicator, and a strong team player with at least one year of nursing experience, dialysis experience, and a current LPN license along with BLS

Qualifications

• Graduate of an accredited school of Nursing.
• Current state appropriate LPN license.
• Minimum of one year of nursing experience.
• Dialysis experience required.
• BLS Provider certification.

Responsibilities

• Provide direct hemodialysis care, perform assessments, administer medications, and educate patients on renal health, treatment, and self-care.
• Delegate tasks to dialysis technicians, supervise their performance, and ensure compliance with clinical standards and regulations.
• Monitor patient response to dialysis, adjust treatment plans as needed, and ensure accurate documentation of interventions and outcomes.
• Work closely with physicians and healthcare team members to coordinate care, report critical lab results, and ensure continuity of treatment.
• Maintain accurate electronic medical records, ensure timely completion of care plans, and adhere to safety and regulatory protocols.
