Augusta Health is seeking a Registered Dietitian/Diabetes Educator to deliver Medical Nutrition Therapy and diabetes self-management education across inpatient, outpatient, and community settings. You will collaborate with the care team, develop individualized care plans, and support at-home continuity of care.

The Registered Dietitian/Diabetes Educator functions as a registered dietitian /diabetes educator in a variety of settings, including inpatient Diabetes Education, the Outpatient Diabetes and Nutrition Self-Management Education program, community events and education, and the Diabetes Support Group. Primary responsibilities include screening, assessing, developing, and implementing individualized nutritional care and education as well as diabetes self-management education to individuals, families, and groups and other patient populations serviced; collaborating with the interdisciplinary health care team to facilitate continuity of care at home and in the community; and maintaining professional competency and skills required for professional practice.
