Job summary

Johns Hopkins Community Physicians (JHCP) seeks a certified Diabetes Educator (Dietitian) to assess, plan, and deliver diabetes education services in an outpatient setting. Under supervision, you will educate patients and families, support transitions from inpatient to outpatient care, and coordinate diabetes-related education for staff and the public.

Qualifications

• Bachelor’s degree in Dietetics is required.
• Analytical ability to gather data, identify problems and refer for resolution.
• Knowledge of medical terminology, diagnosis and basic disease processes.
• Basic knowledge of medical records coding standards and EMR data documentation.

Responsibilities

• Assess, plan, implement and evaluate components of the diabetes education program.
• Provide diabetes education to patients and families in outpatient settings.
• Develop continuing education programs in diabetes and nutrition for professionals and the public.
• Respond to calls about sick day management and transitions from inpatient to outpatient care.
