Job summary

Doral Health & Wellness in Brooklyn, NY seeks a Certified Diabetes Educator (CDE) to provide comprehensive diabetes education and support. You will help patients develop knowledge and skills to manage their condition and improve overall health, working closely with a multidisciplinary team.

The ideal candidate has strong experience in diabetes education, nutrition, lifestyle management, and patient counseling, with a focus on compassionate, patient-centered care and evidence-based practices.

Qualifications

• Bachelor's degree or higher in Nutrition/Dietetics, Diabetes Education, Healthcare, or related field.
• RD/RDN, CDE, or CDCES credential required.
• Experience in diabetes care, diabetes education, nutrition, or a related clinical setting.
• Excellent verbal and written communication skills.

Responsibilities

• Provide individualized diabetes education and counseling to patients.
• Educate patients on diabetes management, nutrition, lifestyle changes, and medication adherence.
• Teach blood glucose monitoring and interpretation of results.
• Guide meal planning, carbohydrate management, and healthy eating habits.
• Support patients in setting realistic diabetes self-management goals.
• Document patient data and progress in EHR/EMR systems.
• Coordinate with the multidisciplinary care team to support care plans.
