Compensation
  • Minimum Offer: $30.60/hr.
  • Maximum Offer: $45.89/hr.

Position Summary

Provide direct and group education for patients with diabetes in a manner consistent with current nationally recognized standards. Assess, plan, develop, implement and evaluate clinical and non‑clinical diabetes educational programs for HSC Operational Components. Ensure adherence to hospital and departmental policies and procedures. Patient care assignment may include pediatric, adolescent, adult and geriatric age groups.

Responsibilities
  • Identify educational needs and issues of the patients; work with the Diabetes Education Coordinator or Medical Director to support educational plans.
  • Ensure the development of instructional programs based on established educational plans. Provide direct and group education for patients with diabetes consistent with current standards and local needs.
  • Develop educational curricula, formulate and/or update teaching outlines, determine appropriate instructional methodologies using adult learning principles to meet the overall goals and objectives of the program.
  • Maintain quality of educational programs by reviewing evaluation summaries regularly, maintaining certifications in relevant specialty areas, and staying current on specialty trends.
  • Serve as an educational resource and role model for clinical and non‑clinical staff.
  • Establish and monitor accurate outcomes for educational programs; revise programs when necessary.
  • Establish communication and consultation relationships with assigned areas, both clinical and non‑clinical staff.
  • Attend meetings and represent the department or hospital in internal or community committees as assigned.
  • Maintain reports and records; collect statistical data for administrative or quality improvement purposes.
  • Maintain a safe, comfortable, therapeutic environment for patients and families in accordance with hospital standards.
  • Enhance professional growth through participation in educational programs, literature review, and attendance at workshops.
  • Follow patient safety policies, procedures and protocols.
  • Proactively identify and seek opportunities to improve patient safety.
  • Identify and correct environmental conditions that may pose risk.
  • Report potential or actual patient safety concerns, medical errors or near misses promptly.
  • Encourage patients to actively participate in their own care by asking questions and reporting treatment issues.
Qualifications

Education:

  • Essential: Bachelor’s Degree
  • Non‑essential: Master’s Degree

Education Specialization:

  • Essential: Related discipline
  • Non‑essential: Related discipline

Experience:

  • Essential: 3 years directly related experience.
  • Non‑essential: Bilingual in English, Spanish, Keres, Tewa, Tiwa, Towa, Zuni, or Navajo with 2 years directly related experience.

Credentials:

  • Essential: CPR for Healthcare/BLS provider or professional rescuers within 30 days; NM RN or NM RD, LD, LN or NM Pharmacist license.
  • Non‑essential: Certified Diabetic Educator.

Physical Conditions:

  • Light work: Exerting up to 20 pounds of force occasionally or up to 10 pounds frequently; may require walking, standing, pushing or pulling. Physical demands exceed sedentary work.
  • Essential: Working conditions: Minor hazard – physical risks, dirt, dust, fumes, noise. Tuberculosis testing required at hire and as needed.
