Job summary

Qualifications

• Nebraska-licensed Health Care Professional (PharmD/RN/NP/PA/APRN/RD) required.
• CDCES credential preferred.
• Experience with health coaching, diabetes education, or disease state management preferred.

Responsibilities

• Lead weekly group education classes with diabetes education and financial coaching.
• Provide intensive health, wellness and financial coaching; manage disease state.
• Document visits comprehensively for study records.
• Support recruitment, screening, data collection and entry as needed.
