Job summary

Baylor Scott & White Health in Austin, TX is seeking a Certified Diabetes Educator to improve patient outcomes through evidence-based self-management education.

100% onsite role focusing on at-risk individuals and diabetes care; teaches self-management, develops programs, and serves as a diabetes care resource within the clinic network. Requires a Bachelor's degree and entry-level experience with BLS certification; RD/RN/RPH credentials preferred.

Qualifications

• Bachelor's degree required.
• Less than 1 year of experience.
• Basic Life Support (BLS) certification within 30 days of hire.
• RD/RN/RPH credentials considered an asset.

Responsibilities

• Plans, directs, and provides individualized self-management education to patients and families.
• Develops educational programs and in-service training for diabetes care.
• Serves as a resource to staff and community on diabetes education.
• Aims to optimize care delivery, patient experience, and outcomes.
• Maintains knowledge of current diabetes care guidelines and education methods.
