Qualifications

• Bachelor's degree required.
• Less than 1 year of experience.
• BLS certification or within 30 days of hire.

Responsibilities

• Plan, direct and provide individualized diabetes self-management education.
• Develop educational programs and in-service training.
• Serve as a resource to staff as a diabetes care specialist.
• Improve care delivery, patient and provider experiences, and safety.
• Apply teaching principles to facilitate self-management.
• Tailor interventions to patient needs.
• Communicate with patients and healthcare team promptly.
• Participate in quality improvement activities.
