Job summary

The successful candidate will be a Registered Dietitian or Registered Nurse with a CDCES certification, demonstrating strong communication and interpersonal skills.

Qualifications

• Registered Dietitian or Registered Nurse in the State of Michigan.
• Two years’ experience working with diabetes patients.
• Current BLS/CPR certification preferred.

Responsibilities

• Conducts assessment to determine patient’s biopsychosocial needs.
• Prepares care plan with short and long term goals.
• Provides counseling on diabetes management.

Education

Registered Dietitian or Registered Nurse in Michigan
